Reimbursement & Coding
Metro is affiliated with many well-known insurance plans and is frequently a preferred provider. Metro also accepts the Medicare assignment; however, it is important to make sure the proper activity level (K level) is assigned during the initial assessment so that the correct components are covered by Medicare. We understand that it can be frustrating to find out that you need to have documentation you were unaware of. The following forms need to be completed by all patients when they are first seen at Metro Prosthetics.
All patients must provide the following:
- A Prescription
- Patient Information Form
- Patient Consent Form
- Minor Consent Form (for patients under the age of 18 only)
The following additional forms are required for a prosthesis:
- Clinical notes from the Prescribing Physician, documenting the patient's need for the prosthesis. Click here for a letter to physicians from CMS (the Agency that runs Medicare), describing what physicians must include in their clinical notes to document the medical necessity of a prosthesis for a patient.
- Signature Attestation Statement from the Prescribing Physician
